Preschool Application Information

We are excited to welcome families interested in our preschool program! Preschool applications will be completed through FinalForms, our secure online registration system.

 Applications Open: March 10

All preschool applications must be submitted through FinalForms before the deadline.

How to Apply Using FinalForms

Step 1: Access FinalForms

Visit the FinalForms website:
 https://sciototech-oh.finalforms.com/

New Families: Create a FinalForms Account

If you do not already have a FinalForms account, follow these steps:

1. Go to the FinalForms website.
2. Click the Parent Icon and select New Account.
3. Enter your:
   • Name
   • Date of Birth
   • Email Address
4. Click Register.
5. Check your email for a confirmation message from FinalForms.
6. Click Confirm Your Account in the email.

Once your account is created, you will be able to register your student.

Returning Families: Log In and Update Information

If you already have a FinalForms account, simply:

1. Go to https://sciototech-oh.finalforms.com/
2. Click Login under the Parent icon.
3. Log in with your existing email and password.

Adding Your Preschool Student

After logging in:

1. Click Add Student.
2. Enter your child’s legal name and required information.
3. Select Yes – New Enrollment.
4. Complete each required form.
5. Type your full name as your electronic signature and submit each form.

Information You May Need

To complete the application, you may be asked to provide:

• Medical history and health information
• Insurance information
• Doctor and dentist contact information
• Emergency contact information
• Preferred hospital information

 Reminder: Applications are NOT COMPLETE until all required documents (birth certificate, Child Medical Statement, immunization records, and Field Trip/Swimming Permission Form) are uploaded.
